Clinical and emotional supportive care in the home to reduce the daily stress of serious illness.


OUR MISSION

We provide supportive care and education to patients and families that improve the quality of daily living for patients facing serious illness even in the earliest of stages. Palliative care focuses on relieving the stresses and symptoms of serious illness and is provided in patient's homes, nursing homes, and assisted living facilities. It can be provided in conjunction with Home Health services and is availlabe to patients of any age, religion, race or illness.


Hospice Versus Palliative Care

Understanding the differences between these two types of healthcare services.


HOSPICE CARE

RN, LPN, CNA, MSW, Chaplain services available and visit frequencies developed upon patient admission to hospice.


24-hour on call for phone triage and nurses make a home visit in the event of an emergency.


Durable medical equipment supplied to patient based off of the RN assessment and safety precautions.


Supplies such as adult briefs and bed pads supplemented for patient and medications managed and refilled by hospice physician.


PALLIATIVE CARE

RN, MSW, and Chaplain services available one visit per month per discipline. Additionall visits on a case by case basis.



All visits occur Monday through Friday between the hours of 8am and 4:30pm.


24-hour on-call nurse available for phone triage only and patient remains under the care of their own physicians.


Medications and changes to medication are managed and refilled by the patient’s own physicians.

PALLIATIVE CARE

Crossroads Palliative Care was established to help patients and families navigate their disease progression as well as the healthcare system. Our goal is to assist the patient and family in making timely and educated decisions in an effort to reduce the burden of symptoms while providing peace and comfort.  We provide three types of care in the home setting which include nursing (RN/LPN) clinical education and guidance, social work support, and spiritual support through our chaplain.

THE PALLIATIVE CARE FOUNDATION

The Palliative Care Foundation of Baton Rouge helps to fund the growing needs of patients, families, and the community by helping provide services to those who are unable to meet all of the financial burdens associated with hospice care. Our goal is to never turn a patient away due to their inability to pay. We also believe in educating the public on hospice and palliative care while also acting as advocates for these pateints.
