OUR VISION
We believe that Home Health care is a well-established extension of medical care, bringing professional services to the infirmed in the comfort of their own home. When it is medically feasible and beneficial, we believe bringing health care professionals into the familiar, comfortable setting of a patient's home allows us to maximize healing by involving the patient and caregiver in the recovery process.
WHAT IS HOME HEALTH?
Home health services are covered by most health insurance plans.
IN YOUR HOME
Home health is available anywhere you call home. It can be in your home, the home of a family member or friend, or even in local senior living or assisted living facilities.
REFERRALS WORK
Any licensed medical provider can refer a patient for home health services. You must meet certain criteria to receive home health services, which your medical provider can determine.
YOU CAN REQUEST IT
You may request a home health provider of your choice. Your provider may have recommendations for home health agencies, but it is always the patient’s choice.
WE COME TO YOU
Based on your medical needs and the medical provider orders, our team comes to you. As a patient of home health services, you have access to an on-call nurse 24 hours a day, 7 days a week.
MEDICAL EQUIPMENT
Home health patients may be deemed to need certain medical equipment or devices to aid in their recovery process. We can facilitate obtaining durable medical equipment for your care.
HOSPITAL COORDINATION
Following a hospitalization, our team coordinates with the hospital to transition you back into your home. We also coordinate with your primary care provider.

HOW WE HELP PATIENTS & CAREGIVERS
We help patients following surgical procedures, such as orthopedic surgery, heart surgery, gastrointestinal surgery and many more. We help patients with learning how to ambulate with an assistive device, such as a walker or cane. We help patients and caregivers follow home exercise plans to build muscle strength and for the patient to perform activities of daily living safely. We help patients and their caregivers navigate through chronic illnesses, such as heart disease, congestive heart failure, diabetes mellitus, COPD, chronic kidney disease, Parkinson's disease, dementia and psychiatric illnesses. We have a nurse on call 24 hours a day, 7 days a week.
